The phrase โcracking the chestโ has understandably shaped perception, but it does not reflect the modern reality of how thoughtfully a conventional sternotomy can be performed.
While minimally invasive and robotic approaches are valuable tools, complex procedures such as multi-valve surgery or combined valve and coronary bypass operations are often best and most safely performed through a traditional approach.
That said, โtraditionalโ does not have to mean more painful.
Through meticulous attention to incision planning, precise midline technique, proactive nerve management with cryoablation, targeted local anesthetic strategies, and the use of softer, more patient-friendly drains, we can significantly improve the postoperative experience.
In my practice, it is not uncommon for patients to report minimal to no pain even within the first week after surgery, regardless of age or procedural complexity.
As surgeons, our responsibility is not only to deliver excellent clinical outcomes, but also to continually improve the patient experience.
Open heart surgery is serious. Pain does not have to be part of that story.
